Your prescription says take two tablets twice daily. Your daily reality is more complicated. The Looms Medication Tracking module is built for that reality — giving patients a complete, organised view of all their current medications, a daily dose logging system and the reminder infrastructure to keep adherence consistent.
Your medication list
Active medications from Looms-issued prescriptions appear automatically. Medications prescribed by providers outside the Looms network can be added manually — name, dose, frequency and start date. The complete medication list is visible to any clinic you share your records with.
Dose logging
Each medication has a daily dose card. Tap 'Taken' when you take your dose, and Looms logs the time. If a dose is not logged within a configurable window after the scheduled time, a reminder notification is sent. At your next appointment, your doctor can see your complete adherence history for any medication.
Medication history
Looms maintains a full history of all medications — current and past — with start and end dates, doses and prescribing providers. This history is valuable context for any new provider and helps identify patterns between medication changes and changes in vitals or lab results.
