Clinic staff spend an enormous amount of time producing repetitive documents — referral letters that say largely the same thing for every patient, health certificates that require the same fields to be filled in manually, discharge summaries that follow the same structure every time. Document Templates in Looms eliminates that repetition.

Building a template

The template builder provides a structured editor where you define the fixed content of a document and mark the fields that should be auto-filled from the patient's record. Patient name, date of birth, diagnosis codes, current medications and attending physician are all available as merge fields.

Rich text formatting, clinic logo placement and digital stamp embedding are all supported in the template editor.

Issuing a document from a template

From any patient's file, select 'Issue document', choose a template and review the auto-populated fields. You can edit any field before issuing. Once issued, the document is delivered to the patient's Looms account with a QR verification code — ready to share with any provider, employer or insurance company.

Template library

Looms provides a starter library of common templates — medical fitness certificates, sick leave letters, referral letters and vaccination records — that clinics can use as-is or customise to match their branding and workflow.
