Who accessed this patient's record? When was this document issued? Which staff member updated this prescription? What's logged
Every significant action on a patient record generates an immutable audit log entry. Logged events include record creation, viewing, editing and deletion; document issuance and verification; prescription creation and dispensing confirmation; appointment creation, modification and cancellation; and all login and access events by clinic staff.
Accessing the audit trail
Clinic administrators can access the audit trail for any patient from the patient's file. The trail can be filtered by date range, event type and staff member, and exported as a PDF or CSV. For compliance auditors, a clinic-wide audit log covering all patients and all staff is available in the Reporting section.
Immutability
Audit log entries cannot be edited or deleted by any clinic user, including clinic owners. This immutability is enforced at the database level. In the event of a legal or regulatory request, Looms can provide a cryptographically verified export of the audit trail for any specified period.
