What This Test Measures

Troponin I and T are regulatory proteins of the cardiac contractile apparatus. They are present in both cardiac and skeletal muscle, but the cardiac isoforms (cTnI and cTnT) are structurally distinct and detected by cardiac-specific antibodies. Heart muscle damage — from any cause — releases troponin into the bloodstream. High-sensitivity assays detect amounts 10–100× smaller than older assays.

When Is This Test Ordered?

  • Chest pain evaluation in the emergency department
  • Ruling out acute myocardial infarction (AMI)
  • Detecting myocarditis, pulmonary embolism, or other cardiac injury
  • Monitoring cardiotoxic chemotherapy
  • Perioperative cardiac risk in major non-cardiac surgery

When Results Are High

Any value above the 99th percentile reference limit indicates myocardial injury. AMI is diagnosed when troponin rises and/or falls with symptoms or ECG changes. A rise from 3 ng/L to 15 ng/L over 1–2 hours is an 'absolute rise' pattern that triggers AMI protocol. Chronic mild elevation occurs in heart failure, CKD, diabetes, and older age — serial testing differentiates chronic from acute.

When Results Are Low

Troponin below the 99th percentile at presentation AND at 1–3 hours after onset of symptoms effectively rules out AMI in most rapid rule-out protocols, allowing safe discharge from emergency departments.

High-sensitivity vs. conventional troponin assays

Conventional assays detect troponin in roughly 50% of healthy people (the assay's imprecision exceeds the detectable signal). High-sensitivity assays detect troponin in over 95% of healthy people — a raised result no longer automatically means heart attack; it means cardiac injury of some kind, which includes infarction, myocarditis, heart failure, and more.

This is why hs-troponin requires clinical context: a 70-year-old patient with known CKD and a stable troponin of 30 ng/L needs different management than a 45-year-old with chest pain and a troponin rising from 8 to 85 ng/L over 3 hours.

Factors That Can Affect Your Result

  • Kidney disease chronically elevates troponin by impairing clearance
  • Sepsis causes cardiac injury and troponin elevation without coronary occlusion
  • Pulmonary embolism raises troponin through right ventricular strain
  • Strenuous exercise (marathon running) can raise troponin above the 99th percentile transiently
  • Skeletal myopathies rarely affect cardiac-specific isoforms but may cause borderline elevation

Tracking This Test Over Time

The critical pattern is the delta: serial troponin measurements 1–3 hours apart. A static mildly elevated troponin suggests chronic cardiac stress (CKD, stable heart failure). A rapidly rising troponin — even from low absolute values — is acute injury until proven otherwise.
