Using your care plan

When a clinic assigns you a care plan on Looms, it appears in your Health Tracking section as a structured document with a title, your condition, goals and a list of steps to follow. This article explains how to read your care plan and mark progress.

Finding your care plan

  • Open **Health Tracking** from the sidebar
  • Select **Care Plans**
  • Tap any plan to see the full details

Reading the plan

Each care plan includes: the condition it addresses, the goals your clinic has set, a list of steps or tasks, a start date, and optionally a review date when your progress will be assessed. Some plans also include notes from your doctor.

Marking steps as complete

As you work through your plan, you can tick off individual steps to track your progress. Your clinic can see which steps you've completed, which helps them tailor follow-up appointments and adjust the plan at your review.

When your plan is updated

Your clinic can modify a care plan at any time — adding steps, changing goals, or extending the review date. You'll receive a notification when the plan is updated. Review the changes before your next appointment so you're ready to discuss any adjustments.

Questions about your care plan

If any part of your care plan is unclear, use the Care Messages feature to message your clinic directly. Attach a reference to the specific step or goal so they can give you a precise answer without needing to reconstruct the context.
