What to do if there's an error in your health record

Errors in health records do happen — a wrong date, a mislabelled document type, a test result attached to the wrong patient. Here's how to handle each situation in Looms.

Correcting a self-uploaded record

Open the record and tap Edit. You can update the title, date, notes and record type. If the underlying document is wrong — the wrong file was uploaded — delete the record and re-upload the correct file. Tap the three-dot menu on the record and select Delete, then confirm.

Errors in centre-issued records

Records issued directly by a medical centre cannot be edited by you — they're marked as verified documents from that provider. If you believe a centre-issued record contains an error (wrong result, wrong date, wrong patient name), contact the issuing centre directly and ask them to correct and re-issue the document. The corrected version will appear in your account automatically.

If you believe there is a serious data error

If you believe your account contains another person's records, or that your records have been mislabelled or altered without your consent, contact Looms support directly. Do not share potentially incorrect records with third parties until the error is resolved.
