Adding personal notes and comments to a record

Every record in Looms supports personal notes — a private annotation that only you can see (unless you explicitly share the record). Notes are useful for capturing context that isn't in the document itself.

How to add a note

  • Open any record from My Records, Lab Results, Radiology or another section
  • Scroll to the Notes section at the bottom of the record detail
  • Tap Add a note or the pencil icon
  • Type your note and tap Save
  • Notes are timestamped and can be edited or deleted at any time

What to include in notes

  • Why the test was ordered — e.g. 'Ordered after chest pain episode on 5 July'
  • What your doctor said about the result — their interpretation, not just the numbers
  • What action was decided — 'Repeat in 3 months', 'Referred to nephrology'
  • Medication changes triggered by the result
  • Your own observations — how you felt at the time, what might have influenced the result
  • Follow-up dates

Notes and sharing

When you share a record via a time-limited link, your personal notes are not included in the shared view by default — only the original document and its AI summary are visible to the recipient. If you want to share your notes with a doctor, you can copy the text into a message or include it in an appointment prep note. For a full guide to sharing, see sharing your records.

Notes on clinic-issued records

You can add personal notes to clinic-issued records (those marked with a verified badge) just as you can with self-uploaded ones. Your note doesn't alter the verified document — it's a private layer on top.
